      Objective To study the effect of ternatolide (Tern.) on expression of granulysin mRNA in the activated peripheral blood lymphocytes (PBLs) of the patients infected dormant tubercle bacilli (DTB). Methods 16kD α crytallin homolog gene of DTB in PBLs of 56 patients was detectd by PCR.Using competitive quantitative reverse transcription polymerase chain reaction (QC-RT-PCR) analyze granulysin gene expression in the PBLs.Results The dosage of 100mg/L and 200mg/L of ternatolide significantly enhanced the granulysin mRNA experession in PBLs in vitro stimulated phytohemagglutinin (PHA) from the patients with DTB (P<0.001).Conclusion The molecular mechanism of Tern. against intracellular pathogens might be associated with the inducting highly level on the expression of granulysin mRNA,a anti bacterial peptide in activated human cytotoxic lymphocytes.

      Objective To explore the role in diagnosis and pathogenesis of tuberculosis of nucleolar organizer regions (NORs) associated proteins (Ag-NORs) by which studies the quantity of Ag-NORs was obtained and the function of T lymphocytes was indirectly reflected.Methods 77 cases with pulmonary tuberculosis,32 cases with tumor and 41 controls individuals were analyzed with a KL Imaging System.Results I.S% of patients with tuberculosis (n=77) and with tumor (n=32) was significantly different with that of the normal control (n=41),P<0.001,while the difference was not found between I.S% of patients with tuberculosis and with tumor (P>0.05).Conclusion The expression activity assay of Ag-NORs can be used to determine the immune function and evaluate prognosis of patients with pulmonary tuberculosis.

      Objective The recombinant 38kD and 16kD protein of Mycobacterium tuberculosis (rTPA38 and rTPA16) as serological diagnostic reagents were evaluated.Methods The purified rTPA38 and rTPA16 were used to detect sera antibodies using enzyme-linked immunosorbent assay (ELISA),and using the PPD as control.Results In detecting the sera of 246 pulmonary tuberculosis patients,the sensitivity of rTPA38,rTPA16 and PPD was 66.3%,63.0% and 72.3% respectively.The specificity of rTPA38,rTPA16 and PPD in detecting of other groups:healthy controls,non tuberculosis respiratory patients,BCG-vaccinated healthy controls,was 97.6%, 96.8%, 86.0%;94.7%,93.1%,75.0% and 93.4%,85.7%,67.9% respectively.In detecting the sera of non tuberculosis respiratory patients and BCG vaccinated healthy controls,rTPA38 had much more positive reactions than that of PPD.To the sera of pulmonary tuberculosis patients,using combination of rTPA38 and rTPA16 as detecting antigens can improve the positive rate by 11.1%.Conclusion rTPA38,which can provide a satisfactory sensitivity and specificity,may become effective antigen in ELISA.A combination of rTPA38 and rTPA16 can improve the sensitivity.

      Objective To understand the value of lipoarabinomannan-IgG and TB-DNA in cerebrospinal fluid (CSF) in the diagnosis of tuberculous meningitis.Methods CSF was the detective specimen.LAM-IgG was detected with enzyme linked immunosorbent assay (ELISA).TB-DNA was detected with polymerase chain reaction (PCR).Results The positive rate of LAM-IgG and TB-DNA in 102 patients with tuberculous meningitis was 51.0%(52/102) and 81.4%(83/102).The positive rate of LAM-IgG and TB-DNA was 91.2%(93/102).But 40 control cases were all negative.Conclusion LAM-IgG and TB-DNA in CSF were useful index for the diagnosis of tuberculous meningitis.

      Objective To investigate the available Methods of differentiate pulmonary tuberculosis from lung tumor,the change of the number of sputum macrophage was determined by using tuberculin test.Methods Tuberculin was injected into subcutis.Sputum was collected before and after the injection.The changes of the sputum macrophage number were measured.Results Before the tuberculin test the average PPD diameter was no significant difference between pulmonary tuberculosis and lung tumor (P>0.05) .After the test the number of sputum macrophage was obviously increased in pulmonary tuberculosis (P<0.05).Conclusion Determining the changes in number of sputum macrophage using tuberculin test was available method for differentiating pulmonary tuberculosis from lung tumor.

      Objective To evaluate the Results of newly registered initial treatment smear positive cases with fully supervised ambulatory chemotherapy.Methods 975 cases of initial treatment smear positive pulmonary tuberculosis registered during 1985~1999 were reviewed and analyzed including treatment regimens,supervised chemotherapy,sputum convert and cohort analysis etc..Results During 1985~1989,the 12 months treatment regimens were applied,the supervised chemotherapy rate was 82%,sputum conversion rate was 93% when the treatemtn courses completed,and cured rate was 86% by cohort analysis.Since 1990,six months short-course chemotherapy regimens,HR(L)ZS(E) were used,the supervised chemotherapy rate was over 97%,sputum conversion rate was over 96%,and cured rate was more than 90% by cohort analysis.Conclusion At present,strictly conducted DOTS strategy,especially fully supervised ambulatory chemotherapy is the best way for control tuberculosis at the population decentralized,relatively poor and extensive rural areas.

      Objective To observe the epidemic trend of drug resistance of the smear positive tuberculosis in 1993~2000.Methods All of the strains of M.tuberculosis from new register smear positive tuberculosis were tested for resistance to INH RFP EMB SM by absolute concentration methods.Results The rate of initial resistance declined from 34.6% in 1993~1994 to 18.2% in 1999~2000.The resistance rate to SM and INH declined from 22.6%,13.2%in 1993~1994 to 13.4%,7.8% in 1999~2000 respectively.The rate of the acquired resistance declined from 52.1% in 1993~1994 to 35.7% in 1999~2000. The total drug resistance rate of MDR was 9.5% in 1993~1994 and 7.6% in 1999~2000.Conclusion The rate of initial and the acquired drug resistance was declined.The total rate of drug resistance of MDR did not decline.

      Objective To reflect the status on case finding in Henan province.Methods Analysis based on the data of the fourth national tuberculosis epidemiology survey in Henan Province.Results Only 5.1% patients were registered and managed in antituberculosis organs;88.6% patients were made a definite diagnosis in non antituberculosis organs,in which,93.6% were not registered.Conclusion To tighten the convergence case management of different medical organizations is the key to increase the rate of case-finding.

      Objective To explore the case-finding procedure with case-finding,high rate easy operated and low costing. Methods The data was collected from the tuberculosis epidemic sampling survey of Shandong Province in 2000. Results 51 patients of smear positive TB,founded by chest X-ray examination and sputum smear test from 1715 suspected TB.Among the patients of smear positive TB,15 patients diagnosed by X-ray examination were normal. Conclusion All the suspected TB. Patients must be examined by sputum test.

      Objective To analyze and compare the changes of the two TB incidences of the sputum smear positive in 1987 and 1998.Methods The investigated Results were analyzed of the two TB incidences of the sputum smear positive in 1987 and 1998.Results The TB incidence of the sputum smear positive was decreased 39.7% comparing 1987 with 1998.Conclusion The statistics and analysis show that the tuberculosis epidemica in Guiyang shows a decline in trend.

      Objective To research the drug fever caused by the antituberculotic medicament in clinical use.Methods 28 cases were analyzed retrospectively.Results Most of the drug fever was occurred within the first two months,occupied 96.4% (27/28),and the streptomycin was the most cause.Conclusion In the course of the chemotherapy,especially the patients with fever occurring in the first two months,the drug fever should be considered so as to avoid any wrong diagnosis or miss any case.

      Objective To investigate infection with Legionella pneumophila of inpatients with tuberculosis and the clinical feature,treatment out come and prognosis.Methods Legionnaires’ disease diagnosed by serological method,treated with exythromycin and RFP (except one who was infected with multi-resistant M.tuberculosis ) for 3~4 weeks.Results 7 patients were found to be infected with Legionella pneumophila through treatment,all patients were cured.Conclusion Patients should also be treated seriously so as to avoid miss-diagnosis though the patients are comparatively less and better cure rate.

      Objective To discuss the causes of misdiagnosis of lung tuberculoma and the indication and pattern of operation.Methods To analyze 108 cases diagnosed as tuberculoma operated and literature reviewed.Results 108 cases were diagnosed as tuberculoma (2.2% of total operated cases in the same period),37 misdiagnosed with 30 cases diagnosed as lung cancer (22 cases with their lesion ≤3cm in diameter),6 benign tumour,1 aspergilloma.40 cases with atypical shadow and with enlarged hilar and mideasternal lymphnodes in the chest film.85 cases (78.7%) lesions located in upper lung field.58 cases (53.8%) had symptoms as tuberculosis.30 cases were operated due to no effective Results by antiTB treatment.Conclusion Based on these results,the possibility and the existence of tuberculoma should be considered in single isolated lung lesion and it is better to have a lung biopsy for pathological examination if possible.The indication and pattern of operation in lung tuberculoma were reviewed.

      Objective To evaluate the therapeutic effect of sparfloxacin (SPLX)on multidrug resistant pulmonary tuberculosis (MDR-PTB).Methods 104 cases of MDR-PTB were allocated at random pair to two groups,using domestic ofloxacin (OFLX) as comparison.52 cases were treated with the regimen of 3SHRZE+SPLX/9HRE+SPLX.52 cases were treated with the regimen of 3SHRZE+OFLX/9HRE+OFLX.Results At the end of treatment,the sputum negative conversion rates of treatment group were 92.3%,while the rates of control group were 69.2%, P<0.05,after two months treatment the difference of the sputum negative conrersion rate in the treatment group was statistically significant when compared with the control group (P<0.01) .Conclusion Sparfloxacin is an effective drug in treating patients with MDR-PTB among other quinolones.
