病灶清除植骨内固定治疗胸腰椎结核的近期疗效分析

摘要/Abstract

摘要： 目的　临床研究同期病灶清除植骨融合内固定术治疗胸腰椎结核的近期疗效。方法将一期病灶清除植骨融合内固定术治疗并随访到的46例,平均随访2.5年,植骨融合按Moon观察标准为主,神经功能恢复情况按Frankel分级进行评价。结果胸腰椎后凸畸形全部矫正(P<0.01)。平均12d下床活动。术后3个月骨性融合率60%;6个月骨性融合率90%。31例并截瘫者神经功能平均恢复3级(Frankel分级);结核治愈率98%。结论一期病灶清除植骨融合内固定术可有效地治疗胸腰椎结核并可早期活动。

Abstract: Objective To evaluate the clinical Results of debridement and fusion with internal fixation to treat thoracic and lumbar spinal tuberculosis.Method Forty-six cases of thoracic and lumbar spinal tuberculosis cases were treated with combination of debridement,fusion and internal fixation.The fusions of all cases were assessed by Moon method and the recoveries of neurological functions were assessed by Frankel grade before and after surgery.Results All cases were follow-up for 2.5 years in average.Kyphotic deformity was corrected(P<0.01).The cases used internal fixation had a higher recovery rate(98%).Conclusion Internal fixation plays an important role in surgical treatment of spinal tuberculosis by early re-constructional spinal stability,correcting deformity,speeding bony fusion,lessening arrest and early functional practice.
