应用硝酸盐还原试验快速检测结核分枝杆菌药物敏感性

Rapid detection of drug susceptibility testing in Mycobacterium tuberculosis by nitrate reductase assay

摘要: 目的评价硝酸盐还原试验(Nitrate Reducrase Assay,NRA)方法检测结核分枝杆菌分离株的药物敏感性的应用价值。方法通过应用NRA方法和绝对浓度法同时检测了492株结核分枝杆菌临床分离株和30株质控菌株对INH、RFP、SM、EMB的药物敏感性,以牛结核分枝杆菌标准株、结核分枝杆菌标准菌株H37Rv做阴阳性对照。并随机选择62株菌株进行重复试验。结果与绝对浓度法比较,NRA检测对INH、RFP、SM、EMB的耐药性的敏感性分别为91.6%、96.5%、92.2%、90.7%;特异性分别为96.0%、98.2%、97.3%、97.3%;一致率分别为95.0%、97.9%、96.2%、96.7%。重复试验符合率为100%。结论NRA是一种成本低、操作简单、快速、与绝对浓度法有较高一致率的药敏试验方法,具有良好的临床应用前景。

Abstract: Objective To evaluate the value of nitrate reductase assay(NRA) for drug susceptibility testing of Mycobacterium tuberculosis. Method Four hundries and ninety-two Mycobacterium tuberculosis strains and 30 standard strains were determined the resistances to isoniazid,rifampin,streptomycin,and ethambutol with absolute concentration method and NRA. Results Absolute concentration method as control,NRA and absolute concentration method in accordance basically(than 95%),NRA susceptibility was more than 90% and specificity than 95%.Compare with absolute thickness law,the sensitivities of NRA for the detection of isoniazid,rifampin,streptomycin,and ethambutol resistances were 91.6%,96.5%,92.2%,90.7%,respectively.The specificities of NRA were 96.0%,98.2%,97.3%,97.3%,respectively.The accordance rates of these two Methods were 95.0%,97.9%,96.2%,96.7%,respectively. Conclusions NRA is a simple,rapid and low-cost drug susceptibility testing of Mycobacterium tuberculosis.
