    Analysis of the National Tuberculosis Control Project in Henan province during the past 4 years

    Objective Reported the evolutional situation of the promoting and strengthing program of tuberculosis control in Henan province.Method According to the program rules,the accurate informations were come from the normal date record and report system which had been built cooperating with the spot supervision.Results Within the past 4 years,29.9% counties in Henan province were executed and cover rates of population was 37.3%.Sputum smear_positive accord rate was 100%,Sputum smear_nagative one was 97% in the quality control test;90.9% of Smear_positive patients had been cured in the cohort analysis(93.1% for initially treated cases and 83.2% for retreated cases).Concolusion The program of Ministry of Health had gained the obvious offect that was less cost and high benefit in the 4 years in plementation and fulfilled the object of the expectant high case finding rate and cure rates.

    Objective To analyse on the implementing effects and influencing factors of tuberculosis control project in the past 6 years.Method The TB control project seasonal and annual report(1992~1997)were collected by central registration department of TB control project office of Chongqing.Result By the ent of 1997,The project coverage rate was 87% in the total population.The total number of examined suspects active pulmonary tuberculosis cases and examined active pulmonary tuberculosis cases were 149 731,431 351 respectively.The smear_positive register rate increased 24 percent point and the average of cured rate of smear_positive cases recently increased 10 percent point than those before the project.These fully verified the technical strategy of the tuberculosis control project is very successful.Conclusion The change of the administrative district,the inadequacy of the counterpart fund,the problem of management of floating population and the transfer of all tuberculosis suspects and cases,etc.,all of these influenced the coverage_rate、the case_finding work and the implementing effects of the tuberculosis control project.

    Objective To evaluate the effect on the promoting and strengthening programme of tuberculosis control conducted by Jiangsu province.Method Analyzing the cure rate of new suptum positive cases at the end of treatment with cohort analysis.Results The cure rates at the end of treatment were 93.8% for initially treated cases and 75.8% for retreated cases,the total cure rates was 91.4%.Conclusion The case finding level and cure rate were improved satisfactory by the project has been conducted and will promoting continuously the tuberculosis control work in Jiangsu.

    Objective To report the monitor of initial drug resistance in new cases of pulmonary tuberculosis during the period 1984—1995 in Xicheng district of Beijing.Method The culture_positive of sputum specimens from the all new cases would be sent to the laboratory center in Beijing TB Control Institute for detection the drug sensitive test.Result The average frequency of initial drug resistance was 14.5%.The main drug resistances were to Streptomycin(11.0%) and Isoniazid(9.8%).47.9 percent of cases were resistant to any one Anti TB drug.The frequency of resistance to both RFP and INH was 1.6%.The frequency of initial drug resistance was related to the age of patient and the severe degree of the disease(P<0.01).There were no relationship between the initial drug resistance and the sex or cavity in X_ray(P>0.05).Conclusion It is one of important contents to monitor the frequency of initial drug resistance in new cases of pulmonary tuberculosis for TB control in a district.At the same time it can evaluate the TB control programme as a main index.

    Objective A group of patients with TB under treatment with DOTS in temporary residence population supervised by voluntary supervisors in a high incidence tuberculosis area in Longgang district,Shenzhen,in order to assist the local TB control program with case holding,and to evaluate the effectiveness of this group of volunteers in enhance complience of notified TB patients to treatment.Methods A cohort study was conducted in 372 cases with TB.Results The volunteers’s group was more compliance to TB treatment than country special group( P=0 024),but it was similar to that of district special group( P=0 730).The patients aged 34 years and younger after intervention with supervision options were significantly improved the comliance than patients aged 35 years and older.Conclusion The supervision option provided by the volureers can improve the comlpliance like district special group.Lower aged adults need more supervision with DOTS.And illustrate the fact that despite the use of DOTS,poor compliance to treatment remains the major barrier to the eradication in China.

    Objective To study the resistance of OFLX of in patients of pulmonary tuberculosis.Methods Retrospective investigation of 205 cases with pulmonary tuberculosis from Jan.1993 to Dec.1995 in our hospital.All cases were sputum culture positive and possessed drug susceptibility test rusults.Results The result showed that 11 cases(17.5%)occurred secondary resistance,2 cases(1.4%) occurred primary resistance.The analysis of patients with OFLX resistant bacilli suggests that long time administration of anti TB drugs and equivalent to monotherapy with OFLX are the causes of secondary resistance and infected by OFLX resistant bacilli may be associated with primary resistance.In addition,the therapeutic effects of OFLX sensitive cases are remarkably superior to OFLX resistant cases.Conclusion OFLX is a new and effective anti tuberculosis drug for it's effect and low rate.

    Objective To study the early diagnosis and reasonable treatment of tuberculous epididymitis.Methods Retrospective analysis of 15 cases of one side tuberculous epididymitis which were confirmed by surgery and pathology. Results Epididymectomy was conducted in 15 and additional orchiectomy in 1.Before surgery 12 cases were misdiagnosed as chronic epididymitis.After surgery sinus was formed in 3 which were cured by anti TB therapy for one half year.No recurrence and remnant of tuberculous was found after 9 months to 7 years follow up.Conclusions tuberculous Epididymitis was easily misdiagnosed as epididymitis.It is helpfull for diagnosis of epididymitis tuberculous by ultrasonographically guided fine needle pucture.Anti TB chemotherapy is the first choice.Operation may be indicated if anti TB therapy is not effective.The remnant of vas deferens must be opened widely on the skin.The incision is not to be drained.Anti TB therapy must be continued after surgery.

    Objective To study on the relationship between clinical and imaging feature in senile sases of tuberculous meningitis with moderate and severe hydrosephalus.Methods Retrospectively analysed 35 senile cases of tuberculous meningitis with hydrocephalus diagnoed by computed tomography(CT) and nuclear magnetic resonance imaging(MRI).Results 82 9% cases with obstructive of aquaeductus cerebri,20% cases with cerebral base cistern adhesion.Increase of cerebrospinal fluid(CSF)protein quantity,Albumin and IgG were relative to hydrocephalus.Conclusion Cerebral membrance fibrin penetration,adhesion and senile stenosis of ventricles of the brain result to hydrocephalus.CT imaging and CSF protein markers were of important value for earlier diagnosis and prognosis.

    Objective To study the pathogenesis of X ray imagings in contrast with pathologic changes in pulmonary aspergillosis.Methods The X ray findings of 51 cases with pulmonary aspergillosis and 28 cases pathologic results among them were retrospectively studied.Results The major X ray findings of pulmonary aspergillosis were as followes(1)parasitic pulmonary aspergillosis:there was aspergilloma shaped like a ball in the cavity of the underlying disease(38 cases).(2)allergic pulmonary aspergillosis:there was the underlying disease in one lobe in lung,and in another lobe there was bronchial mucus cast(“finger cover sign”).(3)invasive pulmonary aspergillosis:there were central necrosis in them.It was original pulmonary aspergillosis and there was no underlying disease in lung.Bronchial arteriography(BAG)demonstrated that there were common hyperplasias of bronchial arterial trunk and its branches(“blushs”),bronchial pulmonary arterial anastomoses(“shunts”),blooding focuses in the pulmonary parenchymal and bronchial arterial aneurysms,and so on. Conclusion (1)Radiographic features of aspergilloma are the important diagnostic bases in parasitic pulmonary aspergillosis.(2)Pulmonary tuberculosis can compicated with aspergillosis,but these tuberculoses were all cured and stable,which suggested that the treatment of pulmonary aspergillesis was the key.(3)Bronchial arteriography demonstrated that the pathologic basis of hemoptysis for pulmonary aspergillosis was the damage of bronchial artery.If embolization for hemoptysis is no effect,sugical resection should be considered.

    Objective To analysis of the identification of Mycobacterium species in strains and drug resistance of Mycobacterium other than Tuberculosis(MOTT) during The periol 1987~1996 in Shanghai.Methods The specific identification of Mycobacteriam and drug susceptibility test were conducted with traditional methods.Results The strains of M.tuberculosis,M.bovis and M.nontuberculosis were 5465(97.3%),22(0.4%),and 132(2.3%)respectively.There were 42 strains of M.tortuitum,30 of M.intrcellulare,17 of M.chelonae subsp,10 of M.scrofulaceum,9 of M.kansnsii,9 of M.gordonae,6 of M.gilvum,4 of M.phlei,2 of M.nonchromogenicum,2 of M.aurum and 1 of M.ulcerans respectively in total of 132 M.nontuberculosis.The rate of resistance to PAS was 72.4%,to INH 61.2%,to SM 49.0%,to RFP 43.9%,to EMB 31.6% and to KM 27.6%.The rate of resistance to only one agent was 13.3%,to two 21.4%,to three 24.5%,to four 16.3%,to five 10.2% and to six 6.1% respectively.Conclusion The pulmonary tuberculosis cases in Shanghai were infected mainly with M.tuberculosis(97.3%)and infected by MOTT only 2.3%,but the high resistance rate of MOTT should be pay great attention.

    Objective To evaluate of Tb NT P BSA antigen in the diagnosis of tuberculosis.Method Sera from 40 tuberculosis cases and 52 healthy controls were assayed by ELISA using Tb NT P BSA(containing the terminal triglycosyl unit of the M.tuberculosis phenolic glycolipid) as antigen.Results The sensitivity and specificity were 94.2%,20.0% for IgG and 79.6%,45.0% for IgM antibody activity respectively.Conclusion This antigen is not sensitive,that will limit its use in the serodiagnosis of tuberculosis.

    Objectives To observe the influence of BCG vaccination on the humoral immunity to tuberculosis bacilli.Methods The serum antibodies to PPD(PPD IgG) of 53 recruits were measured before and after BCG vaccination(12 and 24 weeks later).The OD value of them was compared between 3 measurements.Results Before BCG vaccination,there were 2 cases(3.8%) with positive PPD IgG,At the 12th week after BCG vaccination,there were 15(28.3%)positive;Followed by next 12 weeks,there were 9(17.0%)positive.The OD value of PPD IgG was higher significantly after BCG vaccination,compared with that before(P<0 001).Conclusions The increasing of PPD IgG after BCG vaccination shows that it can boost human beings humoral immunity to tuberculosis bacilli and keep that at a high level.This may interfere with the serological diagnosis of active tuberculosis among children.

    Objective For observation the tolerance of nontuberculosis mycobacteria to anti tuberculosis drugs.Method Ninety strains of nontuberculosis mycobacteria were taken for strain identification and sensitivity test to drugs with traditional methods.Results All strains of non tuberculosis mycobacteria were shown with highly tolerance to PAS,and high to SM,INH,RFP,EBM and less(sensitive)to KM,TH.Among them,the strain of M.Kansasii presented more sensitive to anti tuberculosis drugs.The others such as M.Avium,M.Intracellulare,M.Triviale,M.Terrae,M.Fortuitum,etc.were showed more tolerance to drugs,By the way,the tolerances of non tuberculosis mycobacteria were quite different between high or low concentrations of these six drugs except to the PAS.These explains why a lot of non tuberculosis mycobacteria are resistant naturally to anti tuberculosis drugs.Conclusion To research a new drug and making a further proper therapeutic measure are necessary.

    Objective To assess the clinical value of a rapid immunochromatographic assay based on 38 KDa and other four antigens from Mycobacterium tuberculosis for the diagnosis of patients suspected of pulmonary tuberculosis.Mehod All subjects received sputum smear,culture,serum ICT TB,and chest film examination.Result The total sensitivity of ICT TB test was 45%,specificity 100%.Conclusion This test cannot replace sputum smear,but the sensitivity of the two methods can attain to 63 6% when they are combined.

    Objective To observe the effect of Far advanced pulmonary tubrculosis treated with cell immunity mediator assistant therapy.Method Cell immunity mediator 4ml intramuscular injection qod for 4 months.Results At the end of 6 months,the sputum conversion rate was 91 2% and the foci absorption was 89 5%,catity close up was 81.2%( P<0 05)in the cellimmunity mediator therapy group.Conclusions After fol low up fortwo years,no recurrence case in cell immuntity mediator group.However,3 recurrence cases in the control group(8.8%).The bacteriologic conversion speed in the test group was superior than the control group.
