食管结核164例临床分析

Clinical analysis of 164 cases of esophageal tuberculosis

摘要: 目的 提高对食管结核的认识,减少误诊。方法 分析国内164例经内镜活检或手术标本病理检查确诊的食管结核病例。结果 本组食管结核男女比为1∶1.3,81.9%的患者年龄低于45岁,所有病例均有吞咽困难或胸痛症状,69.5%的病灶位于食管中段。63.2%的患者有结核病史或其他部位结核。术前误诊者占65.7%,其中误诊为食管肿瘤占96.6%。有治疗记录者151例,47例经内科抗结核治疗,104例经手术后抗结核治疗,全部治愈。结论 食管结核好发于食管中段,以中青年女性多见,其主要症状为吞咽困难或胸痛。食管结核易误诊为食管肿瘤,诊断主要根据内镜活检或手术标本病理检查。食管结核预后良好。

Abstract: Objective To improve the recognization and reduce misdiagnosis of esophageal tuberculosis.Methods 164 cases of esophageal tuberculosis published in Chinese language literature including a case from our hospital were summarized and analyised. All diagnosis were established by histology after endoscopic examination or surgical excision.Results There were more females than males(F/M= 1.3∶1) and most of them (81.9%) were below the 45 years old. Almost every one had suffered with dysphagia or chest pain. The sites of lesions were usually middle part of esophagus(69.5%), Only 46 cases(34.3%) were confirmed before surgery by endoscopic examination and most of the others were mistaken as carcinoma (96.6%). Only 151 cases had therapeutic record . All of them were cured after receiving antituberculous therapy and 104 cases of them also receiving thoracotomy.Conclusion Most cases of tuberculosis of esophagus are younger than 45 years old and females were more compared to males. It always occurs at middle part of esophagus and the predorminant symptoms were dysphagia or chest pain. Diagnosis can be difficult even based on endoscopic examination and usually mistaken for carcinoma. Most cases can be successfully treated with antituberculous chemotherapy or combined with surgery.
