中药蛇蝎对颈结核性淋巴结炎患者免疫功能影响及疗效观察

Effect of traditional Chinese medicine (snakes and scorpions) on immune function of patients with neck tuberculous lymphnoditis

摘要: 目的 探讨中药蛇蝎对颈结核性淋巴结炎患者免疫功能的调节作用和临床疗效。方法 60例规范抗结核化疗后复发结核性颈淋巴结炎患者随机分成两组,治疗组 30例,单服中药,对照组30例规范抗结核治疗 (2DRtZE 7DRtE),于治疗前和治疗后各时间段检测外周血T淋巴细胞亚群,并判断两组的治疗效果。结果 两组治疗 6个月后T淋巴细胞亚群比较,治疗组改善 20例,对照组改善 5例,有显著性差异 (P<0.01)。治疗组总有效率 90% (2730),治愈率 53.3% (1630)分别与对照组总有效率 36.7% (1130),治愈率 10% (330)比较均有显著性差异 (P<0.01)。结论 中药蛇蝎对规范抗结核化疗失败的结核性淋巴结炎患者的细胞免疫功能有调理作用,并提高治愈率,在溃疡愈合、淋巴结消肿及改善影像学方面均优于对照组 (P<0.01)。

Abstract: Objective To observe the regulatory effect of traditional Chinese medicine on immune function of patients with neck tuberculous lymphnoditis after receiving regular anti-tuberculous treatment to explore its therapeutic mechanism.Methods Sixty relapse patients with neck tuberculous lymphnoditis after regular treatment with anti-tuberculous drugs were randomly divided into two groups,30 patients in each group.The treatment group were treated with traditional Chinese medicione (snakes and scorpions) only.The control group were treated with routine anti-tuberculous drugs (2DRtZE/7DRtE).T lymphocyte subsets of peripheral blood were determined before treatment and 1~6 months after treatment to compare the therapeutical effect in the two groups.Results The parameters of T lymphocyte subsets were improved in 20 patients within treatment group.After 6 months treatment,the parameters of T lymphocyte subsets were improved only in 5 patients within control group.There is a significant difference between two groups (P<0.01).In the treated group,the total effective rate were 90%(27/30),clinical cure rate 53.3%(16/30).As compared with those in the control group,36.7%(11/30),10%(3/30) respectively,the difference was significant (P<0.01).Conclusion Snakes and scorpions shows the regulatory effect on cellular immune function in patiens with neck tuberculous lymphnoditis failed after regular anti-tuberculous treatment.It could be increased the healing rate.Detumescence of lymphnoditis,healing of ulceration and the improvement of lesion on B-ultrasound in the treated group were better than that of the control group (P<0.01).
