    • Expression of 65 kD Mycobacterial heat shock protein in E. coli

      Objective To construct an engineering strain that can express 65kD mycobacterial heat shock protein. Methods The gene encoding the Mycobacterium tuberculosis 65kD antigen was amplified using PCR and then was cloned in the plasmid. The recombinant plasmid was transformed into E. coli , and then was screened using enzyme digestion and sequencing. The screened recombinant plasmid was transported into E. coli , subsequently the recombinant E. coli was induced by IPTG to express 65kD protein. Results The gene encoding 65kD protein is 1.6kb. Base pairs of DNA sequencing were about 99.1% total gene. The constructed recombinant E. coli can produce 65kD protein of M.tuberculosis. Conclusion E.coli expression system is a rapid and simple means to obtain recombinant antigen of M.tuberculosis.

      Objective To probe the quality control in acid fast stain test of sputum smear. Methods First, quality standards of sputum specimen were worked out to ensure specimen quality. Second, to enhance the quality inspection in sputum smear and put encouragement and punishment into practice. Third, re check regulations to sputum smears after microscopic in spection were formed to ensure the quality of lab report. Fourth, Results of quality control were come out with tabulation to lead to competitive environment. Finally, study on operation rules was reinforced in order to improve quality concepts among laboratory staffs. Results The quality control Results form 1995~2000 showed that these measures were feasible and effective remarkably. Conclusion It is essential that quality control measures be performed in acid fast stain test of sputum smear to improve detective result. Furthermore, these measures must be improved and perfected constantly because the search is far from consummate.

      Objective To study the discrimination diagnosis value of atrial natriuretic peptide (ANP) on benign and malignance lung disease. Methods The levels of ANP in (BALF) were determined by radioimmunoassay (RIA), from 34 cases of lung cancer and 60 cases of benign lung disease. Results The levels of ANP in BALF from patients with lung cancer are significantly higher than those in benign lung disease (P<0.01). Conclusions ANP adopted simultaneously can increase the diagnosis level in differentiating malignance from benign lung disease.

      Objective To assess the characteristic and its clinical value of T lymphocyte subsets in bronchoalveolar lavage fluid(BALF) in patients with pulmonary tuberculosis. Methods T lymphocyte subsets were measured by ELISA assay in BALF and peripheral blood of patients with pulmonary tuberculosis.48 cases were active tuberculosis.19 cases were inactive tuberculosis. Results CD4 T lymphocytes and CD4/CD8 ratios were found in greatly depleted numbers in BALF and peripheral blood in active tuberculosis,there was statistically significant difference when compared with inactive tuberculosis( P<0.01).CD8 T lymphocytes in active tuberculosis patients were high in BALF than in peripheral blood(P<0.05). CD4/CD8 ratios in BALF and peripheral blood were lower in cavitary tuberculosis than non-cavitary tuberculosis. After 2 months’ effective antituberculosis treatments, CD4 T lymphocytes and CD4/CD8 ratios in BALF and peripheral blood showed a significant increase, while CD8 T lymphocytes in BALF decreased. Conclusion Analysis of T lymphocyte subsets in BALF and peripheral blood was helpful for determining the clinical pattern of disease, recovery situation during treatment and prognosis in patients with pulmonary tuberculosis.

      Objective To study radiographic features of spherical pneumonia and to differentiate it from other spherical lesion and reduce misdiagnosis. Methods 33 cases of Spherical pneumonia clinically diagnosed were analysed retrospectively. Results All of the patients’ chest radiographs features:(1)The size of the lesion in nice cases varied from 2 to 4cm (average 2cm), the lesion presented round or oval shape. (2)Most lesions were located in the peripheral lung parenchyma near the plcural surface;(3)The density at the margin of the lesion was lower than that at the central area, sometimes prescnting a halo like appearance;(4)After short term anti inflammatory therapy, most lesions were obviously reduced in size. While the main CT findings were as follows:(1)The lesion presented a triangular shapc;(2)Rough margins with illdefined outline could be senn;(3)Extensive pleural thicken;(4)The lesions were surrounded by small patches of slightly high density and (or) intensified vascular makings;(5)The density of lesion was cvcn, of which had cavity, no calcification.Conclusion Spherical pneumonia has some specific radiographic features, but cometimes radiographic findings of the lesion is similar to peripheral lung cancer and TB, of which diagnosis must be based on clinical feathers and pathology.

      Objective To further investigate the clinical character of tuberculous pleurisy (TP) and the relationship between tuberculous pleurisy and pulmonary tuberculosis.Methods The clinical status of 345 patients with tuberculous pleurisy were retrospectively analyzed.Results ①The age distribution in 345 patients was 51.6% for patients below 35,23.2% over 55. The average age of the overall patients was 38.1. ②Occurrence rate for fever, cough, chest pain was 73.6%, 57.7% and 51.9% respectively. ③Occurence rate for unilateral pleural effusion, bilateral pleural effusion, left and right lateral pleural effusion was 93.0%, 7.0%, 47.8% and 45.2% respectively. ④Moderate and mild pleural effusion was 94.5%, severe pleural effusion was only 5.5%. ⑤The patients with pleurisy occupied 16% for the overall. The patients of tuberculous pleurisy complicated with pulmonary tuberculosis were 47.8%. ⑥The patients with yellow pleural effusion was 89.6%. With bleeding pleural effusion was 10.4%. ⑦The positive rate of M.tuberculosis was 3.2% and 16.8% in pleural effusion and sputum. ⑧The positive rate in PPD reaction was 83.3%. ⑨The increasing ESR was 64.9%, and its mean value was 46.9mm/1h. Conclusion The tuberculous pleurisy was more often occurred in young people. It has close relationship with pulmonary tuberculosis. It is necessary to consider its clinical character in diagnosis.

      Objective To discuss the causes of postoperative complications of pneumoectomy in pulmonary tuberculosis and drscass how to prevent to reducing the complications rate and improve the cure rate of pulmonary tuberculosis. Methods 69 cases of pneumonectomy in pulmonary tuberculosis have been reviewed including 18 cases with serious complications. Results 4 cases were died,2 died of acute respiratory failure, the rest died of diffuse bleeding in the pleural cavity;65 cases were cured including 5 bronchopleural fistula and 1 contralateral dissemiated and other complications. the complications rate is 26.1%(18/69),the mortality is 5.8%(4/69).Conclusion Pneumonectomy is still one of the effective way to treat the patients with pulmonary tuberculosis, but the operation indicationand operation opportunity should be defined; the result shows that unsatisfactory chemotherapy of pulmonary tuberculosis and wrong operative procedure and post-operation inappropriate management are still main causes to operative complications.

      Object To study relationship of asthma and pulmonary tuberculosis. Methods Asthma epidemiological survey in Henan province was done togethen with tuberculosis survery. Result Asthma prevalence was 2.3% in pulmonary tuberculosis patients and 1.0% in Non-pulmonary tuberculosis patients.The asthma prevalence in pulmonary tuberculosis patients was two times than that of Non-pulmonary tuberculosis.The pulmonary tuberculosis prevalence was 1.2% in asthma and 0.5% in Non-asthma patients. The pulmonary tubdrculosis prevalence in asthma was two times more also than one in Non-asthma. Conclusion The pulmonary tuberculosis can be easily complicated by asthma, and asthma can also be complicated by pulmonary tuberculosis. These two are mutual inducing factors.

      Objective To explore the effect of psychologic situation of pulmonary TB patients on the compliance in chemotherapy. Methods To measure and evaluate the pulmonary TB patients’ psychologic situation by means of SCL-90. Meanwhile, the compliance of the pulmonary TB patients was also appraised through analysing and summing up the statistic. Results We tested 70 cases of smear positive pulmonary TB, 28 cases of which proved to be poor compliance as well as mentaly disordered, and the compliance was better after receiving the treatment of both psychologic service and drug. Conclusion The mental disorder was commonly existed among the pulmonary TB patients, and there was found the close connection between the compliance in treatment and mental disorder. To implement the corresponding treatment in accordance with different mental disorder will greatly improve the compliance.

      Objective To analyse the compliance with regular treatment for pulmonary TB. Methods Through questionnaire on patients to investigate and analyse the compliance with regular treatment and its related factors.Results 48.7% of patients were in compliance with regular treatment before registered in.The factors of ir regular treatment were mainly related to the patients’TB knowledge,age and degree of illness,etc.Conclusion It is important to pay attention to the regular treatmen of patients before registered in.DOTS must be carried out.The education work must be reinforced,especially to the elder and/or very sick patients.

      Objective To analyze the effect of different measures for TB control on familial disease financial burden. Methods A survey was conducted of the basic condition?diagnosis and treatment?familial economic state and familial disease financial burden on 499 cured new TB cases from 8 counties covered by the national TB program(NTP) and 7 non-project counties which using conventional TB therapy(CTT). Results The familial disease financial burden in NTP was much lower than that in CTT. Conclusion NTP had achieved better Results not only on case detection rate but also on cure rate, and it was also a better TB control measure having good social benefit and economic benefit.

      Objective To analyse the effect to improve the registration rate and cure rate on TB cases after centralized management of tuberculosis.Methods To collect and analyse all registration data about case finding and treatment of pulmonary tuberculosis cases since the World Bank Tuberculosis Control Project had been carried out in Zhaoqing city of Guangdong province in 1993. Results The smear positive registration rate and new case registration rate of smear positive pulmonary tuberculosis were 17.62/100000 and 7.62/100000 in 1993, and raised to 34.37/100000 and 32.46/100000 in 1999 respectively. The cure rate of newly diagnosed smear positive cases was 95.6%. Conclusion Centralized management of tuberculosis cases in an important procedure to improve the case fiding rate and cure rate.

      Objective To evaluate the effect of TB prevalence and control TB in the She nationality of Fujian Province. Methods The method of Evidence Based Medicine was used to diagnose cases, and taking the cases detect, register, treatment and the cases outcome as the evaluation basis. Results There were 315 active TB cases registered from 2000 to 2001 in 16 She towns, the smear posotive cases were 156(49.5%), among them, the ratio of re treatment’s was 41.7%, and the ratio of young men was 75.9%. The active tuberculosis register rates were 49.2/105, 52.5/105. The new smear positive register rates were 22.0/105 28.3/105. Regular treatment rate was 91.8%. The average cure rate was 90.2%. Conclusion Government’ commitment of supporting the She people as the poor groups was the key to change the backward situation of TB prevention and control in She nationality, and the regular treatment and management was the assurance for improving the rates of detection and cure.

      Objective To evaluate the DOTS effect of new smear positive cases with pulmonary tuberculosis in Baoan District, Shenzhen, 1993~1999.Methods The newly registered smear positive cases with pulmonary tuberculosis including temporary residents in 1993~1999 were supervised.Results 3360 cases with pulmonary tuberculosis were registered in Baoan District, in which 1693 cases were newly registered smear positive. The regular freating rates were 98.19% in permanent residents and 97.5% in temporary resident; the cure rates were 97.4% and 96.7%. There were no significant didfferent between permanent residents and temporary residents (P>0.05). Conclusion The problems of controlling tuberculosis could be effectively solved, as long as the government attaching importance to it, the funds available, completing the three class tuberculosis control network and implementing steadfastly the DOTS measure.

      Objective To estimate the effects of tuberculosis control project in the past 5 years in Yinzhou county, Zhejiang Province. Methods The information of tuberculosis patients was collected according to the program of Ministry of Health. Results 687 infectious patients were diagnosed in the past 5 years. 97% smear-positive of tubercle bacillus patients became negative after 2 months therapy. By the cohort analysis, 97.5% smear-positive patients were cured. The infective periods were significantly shortened by the project. Conclusion Remarkable outcomes have been achieved by 5 years’ practice of the program of Ministry of Health. The goal of high discovery and curing rates has been gained. The tuberculosis control project accords with the rule of low expense and high effect, and enhances the human health.

      tuberculosis control program into primary health care in rural area. Methods The data of tuberculosis control program in Daxing dietrict were collected and analyzed during 1990 to 2000. Results From 1990 to 2000, 77.1% of smear positive cases registered were transferred to district tuberculosis center from the primary health care, 86% of smear positive cases were treated under fully suppervision chemotherapy by health personnel at village or town health center, and 91.2% of smear positive cases were cured. Conclusion Integration of tuberculosis control program into primary health care in rural area can be assured implementing government commitment and modern tuberculosis control program. With strengthening effects of supervision by district tuberculosi center and fully utilizing health resources of primary health care, made the tuberculosis control program sustaining progress in rural area.
