    • Immunogenicity and protective efficacy of a tuberculosis vaccine encoding the antigen MPT64 protein

      Objective To study the immunogenicity and protective efficacy of M.tuberculosis MPT64 plasmid DNA.Methods The mice were immunized with the saline (A),plasmid vector (B), M.bovis BCG (C),and MPT64 plasmid DNA (D),and were then infected by intraperitoneal injection M.tuberculosis strain H37Rv.MPT64 specific antibodies were determined by ELISA.Their lungs,livers and spleens were taken and observed their pathological changes,weighted and performed mycobacterial cultures 4 or 9 weeks after infection.Spleen cells were tested for proliferative responses.Results MPT64 specific antibodies increased in 4 of 10 group D mice.The stimulation rates of spleen cells increased significantly in group D mice compared to those in the other three group mice at 4 weeks after infection.The weights of lungs,livers and spleens in group A,B and D mice were higher than those in group C mice.The spleens CFUs in group A were more than those in the other three groups.The CFUs of lungs and livers had no significant difference among these four groups.The serious lesions could be observed in lungs of group A and B,while not in group C.Group D were different individually.The stimulation rates of spleen cells decreased significantly in group A and B mice,and increased significantly in group D mice compared to those in group C mice at 9 weeks after infection.The weights of lungs,livers and spleens had no significant difference among these four groups.The livers CFUs in group C mice were lower than those in the other three groups mice.Some lesions could be observed in lungs,but not in liver and spleen of each group mice.Conclusions M.tuberculosis MPT64 DNA vaccine,which was immunized mice by intramuscle injection,could induce specific humoral and cellular immune response and provide some degree of protection against Mycobacterium tuberculosis challenge.

      Objective To analyze the comparability between proportion method and absolute concentration method in drug susceptibility testing (DST).Methods 453 isolates from nationwide epidemiological survey were subjected to susceptibility testing against 4 anti tuberculosis drugs using both proportion method and absolute concentration method.Results The resistance to each of 4 drugs in 402 M.tuberculosis isolates did not show statistically significant difference between two methods.In 51 NTM isolates,only was the resistance to EMB significantly higher in proportion method than in absolute concentration method.The accordance rate between two Methods was 96.2%、97.1%,98.7% and 96.9% for INH,SM,RFP and EMB respectively.Conclusion The accordance between proportion method and absolute concentration method is high,and the Results obtained by using proportion method are comparable with both international standard and previous data of our country.

      Objective To study clinic characteristic of pulmonary tuberculosis related AIDS.Methods Retrospective analysis infection ways, clinic symptoms, examination and diagnosis, and treatment condition to 21 tuberculosis cases that were tuberculosis related HIV infection.Results 95.2% of which was youth-prime of life in 21 cases, scoped to 71.4% (15cases)by blood contact transmitted way and 42.9%(9cases) by sexual. 19%(4cases)by both .Clinic present continued fever (100%), cough (61.9%) and diarrhoea (33.3%) over one month. PPD test negative rate was 81.9%, lymphnode tumour 61.9%, Chest X-ray film showed military tuberculosis was 76.2%, lymphnode tuberculosis of out in pulmonary was 52.4%, fatality rate 61.9% in a year of half. Conclusions It is different that only pulmonary tuberculosis and tuberculosis related AIDS. PPD test negative rate is higher, X-ray film is atypical , Disease situation is more complex, treatment result is no good, fatality rate is higher that pulmonary tuberculosis related AIDS than only pulmonary tuberculosis.

      Objective To know register and cure cohort situation of TB in Fujian province in the past decade,and provide the technology policy of continuable develop for TB control. Methods To register case and cure cohort analysis according the report forms of TB register from 1991 to 2000. Results From 1991 to 2000 total new smear positive cases were 28391,the proportion of smear positive cases in activity cases was raised from 25.3% in 1991 to 37.4% in 2000,the new register rate of smear positive cases were raised from 7.3/100000 in 1991 to 19.2/100?000 in 2000,cure rate was raised from 80.0% in 1992 to 89.2% in 2000.Conclusion Fujian province is getting the anticipative effect in the control of TB.

      Objective To discuss the indication and opportunity of the pulmonectomy for massive hemoptysis.Methods Retrospectively analyzed the therapeutic effect of 32 cases with massive bemoptysis’ treated by pulmonectiony from 1985 to 2000.Results The cure rate of 32 cases with massive hemoptysis’ was 87.5%.Conclusions The operation indication is as follows:the bloody operation position is definite,the lung function and the general condition are better.The surgical showled be conduct as early as possible,especially,The operation indication should be seriously controlled,because the complication and death rate are higher than that of the routine pneumonectomy.

      Objective To study the treatment pattern of tuberculos pleural effusion.Methods This study using minute tube drainage new techniques,42 cases were use drugs empties into the thoracic cavity.The control group using classical treatment model.Regular antituberculosis treatment was to all patients observed.Results There were 42 cases after treatment in cavity inside reached control.The efficacious ratio was 100% (42/42).93%(39/42) of the 42 cases achieved complete improvement and another 7%(3/42) of the 42 cases achieved partial improvement.The control group were efficacious ratio was 86%(36/42).76%(32/42) of the 42 cases achieved complete improvement and another 10%(4/42) of the 42 cases achieved partial improvement.14%(6/42) improvement.Conclusion 42 cases tuberculosis pleural effusion in thoracic cavity was a convenient safe method that can be used widely.The treatment outcome as presented in this article was very good and the treatment model was valuable for clinical practice.

      Objective To explore the causes of circular lump produced after the treatment of tuberculous pleural effusion (TPE).Methods The discovering time,localities of lump and curative effect were analysed retrospectively in 33 cases of TPE.Results The lumps were circular or oval,closing the sick side of the chest,mostly produced in the intensire stage of chematherapy and disappeared while the chemocherapy continusly.Conclusion The cause of the forming of circular lump may be allergic reaction in the treatment of tuberculosis.

      Objective To analyze the main cause of high sputum smear positive vising rate during the past years.To evaluate the effection of smear positive TB control,To make science basement for TB infection control.Methods 8609 cases of smear positive pulmonary tuberculosis registered during 1991~2000,7465 cases of new smear positive pulmonary tuberculosis and the statistic and analysis were adopted,which showing the TB epidemic in Yunnan province.Results The smear positive rate shows a rise trend,the average rise rate is 4.8% during 1991~2000.The male smear positive rate is higher than that in female ( P<0.05);The smear positive cases is high in 25~34 years old group,7465 new smear positive cases are found in the 10 years,which is only 10.5% of that should be found,the cure rate is 74.3%.Conclusion The low cure rate and low found rate is the main cause of high smear positive rate.So the government admit the administrative intervention and implement TB technical control measule to reduce the TB epidemic situation.
