摘要: 布鲁杆菌侵袭脊柱引起椎间盘炎或椎体炎时称为布鲁杆菌病性脊柱炎(brucellar spondylitis,BS)。早期可通过影像学、实验室检测结合临床表现准确诊断;其中,BS主要需要与脊柱结核相鉴别。明确诊断后需进行联合、规范、足量、足疗程用药控制BS。对于单纯药物治疗及保守治疗效果差、符合手术适应证的患者,可在药物治疗的基础上,根据病情采取前路、后路或前后路联合手术,彻底进行病灶清除,同时使用内固定装置增加脊柱的稳定性,以便更加有利于症状的改善,防止疾病复发。对于微创手术治疗BS,应严格按照手术适应证制定手术方案。目前,BS的诊疗工作虽已取得长足的进展,但深入的研究、制定规范化的诊疗标准仍必不可少。

Abstract: Brucellar spondylitis (BS) occurs when Brucella attacks the vertebral column and causes inflammation in the intervertebral disc or vertebral body. Early diagnosis can be conducted by imaging, laboratory and clinical manifestations. It is mainly needed to be distinguished from spinal tuberculosis when diagnosis. BS should be controlled by combined, full-dose and full-course medical therapy. If the outcomes of medication and conservative treatment are not satisfactory and the patient is in conformity with the operation indications, operations via anterior, posterior or combined approach should be chosen according to the patient’s condition. During the operation, the lesion should be completely removed and the internal fixation device should be used to increase the stability of the spine, which is benefit to improve symptoms and prevent recurrence of the disease. The surgical procedures should be strictly planed with the indications for surgery when the surgeon chooses minimally invasive surgery for BS. At present, although the diagnosis and treatment of BS has made great progress, it is still necessary to study in depth and formulate standardized standards of diagnosis and treatment.
